* Students in Primary, Secondary, ITE, JC, POLY or University only.
 

Camp Rates are inclusive of:

  • 4D3N stay at the Putrajaya Marriott Hotel, KL, Malaysia
  • Buffet Breakfast, Lunch and Dinner
  • Bus transportation

Early Registration (including full payment) ends February 26, 2012
Final registration and 50% deposits must be made by March 11, 2012
What you need to sign-up:

  • Complete your registration details below
    Parents, please complete one registration form for each family member that is attending.

  • Register Your Family Members Individually
    If you are going as a family, you will need to register all your family members separately. Make a note in the remarks field below letting us know who your family members are.

  • Make a 50% deposit
    A 50% deposit of your total fee(s) is required for registration on the following Sunday after completing this form. Look for us at the Visitors Table after service.

  • Email a scanned copy of your passport
    Send a scanned copy of your passport and any accompanying family members/helpers to amy.burton@everynation.org within 3 days of completing the registration form.
    (a scanner will be available at the Visitors Table after service.)

  • Parents & Legal Guardian's Consent
    If you are under 21 years old, you must submit a release form signed by your parent or legal guardian avaible at the Visitors Table along with your deposit.

  • Employer's Consent
    If you are a domestic helper working in Singapore, you must have your employer complete the release form available at the Visitors Table and submit it along with your deposit.
